Waiver

THIS FORM MUST BE READ AND SIGNED BEFORE THE PARTICIPANT IS ALLOWED TO TAKE PART IN ANY LAZER RUNNER EVENT.

IN CONSIDERATION of being permitted to participate in any way in the sport and activities of Laser Tag under the auspices of LAZER RUNNER, I acknowledge, appreciate, and agree that:

1. Acknowledgement of Risks: I understand that the risk of injury from the activities and equipment at LAZER RUNNER is significant, including the potential for permanent disability or death. While specific protective equipment and personal discipline can minimize these risks, the possibility of serious injury does exist.
2. Assumption of Responsibility: I knowingly and freely assume all risks, both known and unknown, including those arising from the negligence of the individuals or entities released from liability below, and I accept full responsibility for my participation.
3. Understanding of Activity Intensity: I acknowledge that Laser Tag activities are physically and mentally demanding. I agree to comply with all rules and regulations. If I notice any unusual or unnecessary hazards during my participation, I will promptly inform the nearest official.
4. Health and Medical Condition Acknowledgment: I certify that I am physically fit and have no medical conditions that would prevent my safe participation in Laser Tag activities. If I have any health or medical concerns, I have discussed them with my physician before participating in any Laser Tag activity.
5. Code of Conduct Agreement: I agree to conduct myself responsibly and to follow the instructions of staff and officials during all Laser Tag activities. I understand that disruptive or unsafe behavior may result in my immediate removal from the premises without refund.
6. Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless LAZER RUNNER, the owners and lessors of the premises, their officers, officials, agents, and/or employees (“Releasees”), from any and all injury, disability, death, or loss or damage to person or property, whether caused by the negligence of the Releasees or otherwise.
7. Scope of Agreement: I understand and agree that this Release of Liability Agreement applies to each and every Laser Tag activity or event I participate in at LAZER RUNNER from this date forward.
8. Consent for Media Use: I acknowledge that my participation in LAZER RUNNER activities may be recorded via photos, videos, or other media. I give permission for LAZER RUNNER to use such media in advertising, promotional materials, or social media without any compensation. I understand that this media may be publicly available.

Certification of Understanding: I have read this Release of Liability and Assumption of Risk Agreement, fully understand its terms, and acknowledge that I am giving up substantial rights by signing it. I sign it freely and voluntarily without any inducement.

Parent/Guardian Consent (for participants under the age of 18): This certifies that I, as the parent/guardian with legal responsibility for this participant, consent to his/her release of LAZER RUNNER and all other Releasees. Additionally, I agree to release and indemnify the Releasees from any and all liabilities arising from his/her involvement in these activities.
